Here's one example: in a claim recently filed, the client had total knee replacement surgery that had been preapproved and cleared by the insurance company. About a month after the surgery, the insurance company revoked its approval, refused to pay for the surgery and related expenses, refused to pay for the continuing physical therapy and stopped paying any temporary total disability benefits. It will work out in due course but only after unnecessary and wrongful harm done the client.
