What is the OSHA fine proposed after repeat violations led to the death of two oil rig workers? $93,700.  The worker, Sandy Daves and Jason Bolt, were killed when a piece of equipment they were using came apart.  Among the violations by Robinson Drilling of Texas Ltd. were failing to follow industry practices that could have prevented the accident, failing to provide covers on floor holes to prevent trip hazards, not ensuring that breathing equipment was fully charged and not making sure work surfaces were clean and dry.
