Multiple and egregious safety violations have been cited against a Kansas City bus operator related to a fatal accident in which a rider was ejected through unsecured double doors.  Jamie Frecks, 26, a single mother with an infant, was thrown from a party bus through double doors that had been used to accomodate wheelchair-bound riders, according to wrongful death lawsuits filed on behalf of her infant child and her estate.  The bus operator, Midnight Express, did not have a U.S. Department of Transportation (DOT) certificate to operate and probably did not have the $5 million in liability insurance that goes along with such certification. DOT issued a cease and desist order against the company following Frecks' death.
