A workplace forklift accident that killed a worker, a 17 year old male, has resulted in an OSHA fine of $16,000.  The workplace accident happened at a meatpacking plant.  OSHA found serious violations in inadequate training of forklift drivers, inadequate supervision of forklift trainees and irregular use of seat belts.
