Compounding and specialty pharmacies show a pattern of safety violations as well as lax state and federal government oversight and regulation, according to a report in the Washington Post, Compounding Pharmacies Have Been Linked to Deaths, Illnesses and Safety Violations for Years. The safety failures predate the meningitis outbreak last fall that killed 45 people as a result of violations at the New England Compounding Center.
