Inadequate leading to the death of two nursing home residents has led to fines totaling $180,000 reports the Los Angeles Times, "Two Orange County Nursing Homes Fined for Patient Deaths."  One death was the result of dehydration, the other by a failure to resuscitate where the nursing home staff mistakenly believed that orders existed not to resuscitate the resident.  The fluids records of the patient that died of dehydration were blank or illegible.
