A nursing home has been fined for providing "substandard care" to numerous residents; three of the 26 violations presented "immediate jeopardy" to residents. The most serious violations at the Syracuse, New York nursing home involved a feeding and a failure to supervise properly residents in danger of choking. There were also a number of repeat violations.
