State public health officials are reporting that the New England Compounding Company shipped orders of the steroid at the core of the nation-wide meningitis outbreak without waiting for sterilization testing confirmation, a basic safety violation.  Some are saying that the meningitis outbreak was preventable but for the failure to follow proper safety procedures.
